To help ensure a productive visit:
- If available, please bring X-ray or imaging studies performed within the last 6 months pertaining to your orthopaedic condition. This will allow the provider to conduct a thorough and comprehensive evaluation
- Please bring the name, address and phone number of your primary care physician (PCP), other specialty physician(s) involved in your care, as well as the contact information for your dentist
- You will need to bring medical records from prior visits to other physicians that you think may be helpful
- Please bring information about prior treatments and medications that have been tried in the past
- You will need to bring your current insurance card(s) and driver's license
- Please obtain a referral authorization from your primary care physician if required by your insurance carrier prior to your appointment. If your appointment requires an authorization due to other circumstances, this must be obtained prior to the visit as well
- Patients who do not have insurance are required to pay at the time of service (payment arrangements can be established at the time the appointment is scheduled)
- Please bring a list of questions you want to ask your physician or practitioner
We kindly request that you contact our office 24 hours in advance if you are unable to keep your scheduled appointment time so that we may offer the opening to another patient
